Overview
The Group Living Portal provides care home managers and multi-disciplinary teams with a consolidated, AI-powered view of health and wellbeing across all residents at a group living location.
By aggregating data from telecare call transcripts, the portal identifies cross-resident trends, escalating risks, and generates professional-grade assessments from five clinical perspectives — enabling proactive, coordinated care interventions before issues escalate.
Benefits
Early Warning Detection
AI continuously analyses call transcripts to flag residents whose health indicators are deteriorating, enabling early intervention before a crisis occurs.
Cross-Resident Trend Analysis
Spot patterns across the entire location — such as a cluster of respiratory symptoms or a rise in fall-related calls — that might indicate environmental or seasonal factors.
Multi-Disciplinary Assessments
Each resident receives AI-generated assessments from five professional perspectives: Nurse, Mental Health Specialist, GP, Social Worker, and Physiotherapist.
AI-Generated Action Items
The portal produces categorised manager actions — immediate, preventive, briefing points, and referrals — so care teams know exactly what to prioritise.
Resource Allocation
Understand which residents need the most attention, enabling managers to allocate staff time, specialist visits, and resources where they will have the greatest impact.
Coordinated Response
Brings all resident intelligence into a single dashboard, replacing fragmented notes and verbal handovers with a structured, shareable overview.
How It Improves Care
Spotting Escalating Risk
A resident who mentions increasing pain across several calls may not trigger concern from any single transcript. The portal aggregates these mentions and flags the escalation, prompting a GP review before the condition worsens.
Outbreak & Cluster Detection
If multiple residents report similar symptoms (e.g., coughs, stomach complaints) within a short timeframe, the trend analysis highlights this pattern — alerting managers to a potential infection outbreak requiring immediate environmental or hygiene measures.
Staff Briefings
The manager actions section generates ready-to-use briefing points, making shift handovers faster and more comprehensive. New or agency staff can quickly understand the current state of each resident.
Proactive Interventions
Rather than reacting to emergencies, care teams can use the preventive action items and professional assessments to schedule wellness checks, adjust medication reviews, or arrange specialist referrals ahead of time.
Getting Started
- Navigate to the Group Living Portal tab in the main application.
- Select a location from the dropdown menu. Locations are group living addresses with two or more residents who have call transcripts on file.
- Click Load Dashboard. The system will aggregate all resident data for that location and generate the AI analysis.
- The dashboard loads with health alerts, charts, resident directory, trend analysis, and manager actions.
Understanding the Dashboard
Health Alerts
Critical and warning banners appear at the top when the AI identifies urgent situations requiring immediate attention, such as a resident with a rapidly deteriorating condition or a safeguarding concern.
Symptom Overview Chart
A horizontal bar chart showing the most frequently mentioned symptoms across all residents. Helps identify common health themes at the location.
Call Volume Timeline
An area chart displaying call frequency over time. Sudden spikes may indicate a period of increased distress or a developing situation that warrants investigation.
Risk Distribution
A donut chart breaking down how many residents fall into High, Medium, and Low risk categories, giving an at-a-glance view of the overall risk profile for the location.
Resident Directory
A grid of resident cards showing each person's name, ID, risk level (colour-coded), and key concerns. Click any card to view their detailed professional assessment.
AI Trend Analysis
A narrative summary identifying cross-resident patterns, emerging risks, and noteworthy changes over the analysis period. This is generated by AI from the aggregated transcript data.

Overall Summary
An AI-generated narrative summarising the resident's current state across all professional domains, highlighting the most pressing issues.
Immediate Actions
When the AI identifies urgent needs for a specific resident, these are listed as prioritised action items that should be addressed promptly.
